Required documents:

Lithuanian passport or identity card;
Child’s Lithuanian passport or identity card. If Lithuanian passport, Lithuanian identity card have not been issued, birth certificate is submitted. If a child or children live in Lithuania, it is mandatory to provide the information who takes care of a child and where a child lives.
